UXO Impedes Response To Helicopter Crash

Pine Valley, California A Marine AH-1 Super Cobra helicopter crashed in a remote area of Cleveland National Forest with ordnance onboard including live 2.75 inch rockets. Local firefighters, the military and the U.S. Forest Service worked together to respond to the crash. Due to the explosive hazard caused by the ordnance, the response team was restricted from entering the area of the crash site, which spanned approximately 2-acres.

A civilian helicopter was used to make water drops over the crash site while a Marine EOD unit was dispatched to the accident site to clear the area of UXO. The amount and type of UXO recovered from the scene by EOD was not reported.

The helicopter that crashed was from the 3rd Marine Aircraft Wing based at Marine Corps Air Station Miramar. The helicopter crashed approximately 40 minutes after taking off from a training mission out of El Centro. The two pilots on-board at the time did not survive the crash. The cause of the accident is under investigation.

Man Finds Artillery Projectile During Garage Clean Out

Colorado Springs, Colorado A man clearing out an old garage came across a large artillery projectile among the collection of material in storage. The alarmed man called the police who responded with bomb technicians from the Colorado Springs Police Department. The bomb technicians called Fort Carson EOD for support after verifying that the round was a military munition. EOD responded and identified the round as a "live" 152mm projectile. Reportedly, the round was purchased at an Army surplus store nearly 35 years ago from a previous tenant. The exact type of live 152mm projectile was not reported but it was reported that the round had never been fired. EOD safely removed the projectile from the garage and properly disposed of it.

Bomb Dredged Up During Beach Replenishment

Sutton, United Kingdom A dredger pulling sand from the bottom of the seabed 4-miles out from shore discovered a munition item in the process. The captain of the vessel called the Yarmouth Coastguard who responded with technical support from the Royal Navy EOD Team based out of Portsmouth. EOD boarded the vessel while the 22-man crew was safely evacuated. EOD rendered the item safe before taking it to shore for proper disposal. No injuries were reported and the crew was allowed to return to work after the incident.

Military Eliminates UXO with German Engineered Training Ammunition

PEO Soldier's Project Manager Soldier Weapons in Picatinny Arsenal, New Jersey has three 40mm product standouts in the Green Ammunition category. Yes, that's Green Ammunition.These training cartridges enable 24/7 day/night combat training on the high velocity MK19, MK47 grenade machine guns and low velocity M203, MK13, M79 and M320 grenade launchers. The MK281 MOD 0 and MOD 1 are for high velocity, and the M1110 is for low velocity.

Commanders and warfighters that have used the cartridges value the realistic fire and maneuver training they deliver, the good hit signatures,and the convenience of unrestricted training in dry conditions with no cease fires associated with pyrotechnic cartridges. Warfighters have, or will have, access to this ammunition. That's because Project Manager Soldier Weapons leveraged the DoD's Foreign Comparative Test (FCT) program to fast track qualification of a commercial-off-the-shelf (COTS) product used in a NATO country.

Stray Shots From Range Strike Buildings Near Nuclear Power

Lusby, Maryland Five small arms bullets strayed from a training range located approximately .5 miles from the nuclear reactor towers on the Calvert Cliffs Nuclear Power Plant during a Maryland State SWAT Team training exercise. No injuries were reported as the bullets hit two separate buildings and an unoccupied vehicle located thousands of feet from the range. One of the five bullets hit a window in a building causing one of its panes to shatter.

Plant security officers use the small arms range on the grounds of the power plant approximately 200 days a year. The range is also used regularly by county sheriff's deputies and by SWAT teams during a major annual training exercise. The incident occurred during the recent annual SWAT training exercise. SWAT members typically fire in a southward direction away from the plant but in one particular exercise, officers fired eastward toward the plant. When this occurred, five of the rounds fired cleared the 30-foot high berm and proceeded off-range.

Plant officials conducted an investigation and reported the incident to the NRC. Plant officials did announce that at no time were the reactors in any danger of being compromised. Ballistics tests are underway to determine which officer(s) fired the shots that went astray.

Livestock Recover UXO

Wiltshire, United Kingdom a farm worker feeding a herd of pigs noticed the tail fin of a munition protruding out of the ground. Apparently the pigs had uncovered the UXO as they were rooting around in their feeding area. The farm worker reported the find to the farm owner who in turn called the police. The police responded with technical support provided by an 11th Regiment Royal Logistics Corps EOD unit.

EOD identified the UXO as a 1960s era unexploded mortar round. EOD determined that the item was unsafe to move so an in-place disposal detonation was performed. Local traffic around the farm was diverted during the disposal operation. Reportedly the farm was once part of a military training area back in the 1960s. No injuries were reported and the pigs were released back into the area following the successful disposal operation.

WWII Era Bomb Found in Popular Diving Area

Marsascala, Malta Recreational divers diving on Munxar Reef found a large bomb on the seabed in approximately 35 feet of water. The divers reported the find to the police who contacted the EOD dive team from the Armed Forces of Malta Ammunition and Explosives Company (3rd Regiment). EOD responded identified the bomb as a WWII era German HE filled Sprengcylindrische (SC) general demolition bomb.

The unexploded bomb which weighed over 2,000 pounds was raised to the surface with the use of air-lifting bags. Once the bomb was on the surface, EOD conducted a render safe procedure and subsequently towed the bomb further out to sea for open detonation disposal.

The German SC series of bombs used heavily during WWII are made of thin steel with a comparatively heavy pointed nose welded onto the bomb. The rear ends of SC series bombs are threaded to accept a female base plate. The tail attachment brace is tack-welded to the body just forward of the base plate. SC bombs are loaded with either cast TNT, powdered amatol, or trialen.

Corporate Environmental Safety and Health Manager (CESHM)

The PIKA CESHM will be responsible for the overall management of the PIKA Corporate Environmental Safety and Health Program (CESHP) and for the effective implementation of the regulatory requirements associated with the CESHP. This will include the effective integration of environmental, safety and health (ES&H) values and initiatives into all phases of project acquisition, design, planning, management and implementation.

ESSENTIAL FUNCTIONS
  • Manage, review, and implement the PIKA CESHP.
  • Write, review and approve Accident Prevention Plans, Site Safety and Health Plans, and Activity Hazard Analysis forms for multi-disciplinary, multi-faceted, environmental remediation, building decontamination and demolition (D&D), and explosive hazard mitigation projects.
  • Provide consultation and recommendations to project management personnel during all phases of project execution.
  • Conduct near miss, incident, injury, or illness investigations to discover root causes and develop corrective actions.
  • Conduct site visits to evaluate site and work hazards prior to bid or plan development.
  • Conduct periodic on-site audits of projects to determine their compliance with the PIKA CESHP.
  • Develop air monitoring and sampling protocols for projects where airborne hazards may be present.
  • Coordinate with PIKA Human Resources Department regarding training, medical surveillance, drug testing, and recording keeping requirements.

Navy EOD Technician Killed in Iraq

Petty Officer 2nd Class Tyler J. Trahan, 22, of East Freetown, Massachusetts died along with two Marines while conducting combat operations in Fallujah, Iraq. Tyler was assigned to the Navy EOD Mobile Unit 12 based out of Norfolk, Virginia but was on a temporary deployment with a Navy SEAL team when he died. The Navy has not released any details surrounding the deaths. Tyler is the 12th Navy EOD technician killed in Iraq combat operations.

Man Decides to Keep Grenade Before Having Second Thoughts

Redlands, California A 26-year old man found a grenade near an auto parts store. Instead of calling the police, the man decided to keep it as a souvenir. However, a couple of hours after finding the grenade, the man changed his mind and stashed the grenade in some bushes next to a parking lot for a fast food restaurant. He then called the police to report the grenade.

Police responded along with technical support provided by the local Sheriff's Department Bomb Squad. The Bomb Squad used their robot to retrieve the grenade from the bushes and placed it on the parking lot where it could be inspected. After further inspection, the Bomb Squad determined that the grenade was inert. Authorities are unsure how the grenade ended up near the auto store in the first place.

Description
The 4.2 inch M335A2 Illumination mortar is used for target and battlefield illumination at night and during other periods of low visibility. The complete round consists of a projectile body with a detachable base plug, a time fuze, an illuminant assembly attached to a parachute assembly, and a tail assembly. The steel tube body is designed to accommodate an expelling charge immediately below the fuze, and the base plug is attached with four equally spaced shear pins. The illuminant assembly consists of a first-fire charge and an illuminant charge, contained in a canister fitted with anti-rotational brakes to reduce canister spin at the time of ejection and prevent twisting of the parachute suspension lines. The tail assembly includes a pressure plate and rotating disc, a propelling charge, a cartridge container and ignition cartridge, and a striker nut assembly.
